Overview

How do you participate in the reimbursement program?

Any employee, spouse or dependent (over 13 years), who are eligible and/or enrolled in Lucas County Benefits are eligible to participate.  Activities eligible for reimbursement include: approved gym memberships, weight loss programs, smoking cessation, cardiac rehabilitation, prescription weight loss medication, or diabetes case management. Other activities may be eligible for reimbursement on a case-by-case basis. Employee Benefits must approve these activities in advance. In order to receive reimbursement, you must also provide documentation of your attendance record and receipt of payment.

Terms and conditions apply to each type of reimbursement. 

Fitness Membership/ Weight Watchers

Upon successful completion of the program requirements, you are eligible for up to 50% reimbursement for the activity cost. For fitness membership/classes reimbursement, you must attend a minimum of 52 days in six months. For Weight Watchers you must attend 10/12 meetings in a 3-month, 20/24 meetings in a 6-month period.

In addition to fitness reimbursements, other exercise classes such as ZUMBA, Body Sculpting, Bootcamp, Pilates, and Yoga may also be eligible for reimbursement.

Fitness memberships and/or weight loss programs may be combined for a maximum annual benefit of up to $400 for an individual contract and up to $600 for a family contract.  To be eligible for a family reimbursement, two members of your family (age 13 and over) must both meet attendance requirements.

Smoking Cessation Program

The Wellness Program reimburses 100% of the cost of smoking cessation methods. Cessation methods can include: nicotine patches, nicotine gum, prescription nicotine replacement medication, hypnosis, and acupuncture.

You are eligible for reimbursement up to a two-time maximum reimbursement of $195.00 per eligible person.

Cardiac Rehabilitation Program

Enrollees participating in Phase III and IV of an approved Cardiac Rehabilitation Program will be reimbursed at 100% of the program cost. Reimbursement is not limited to the maximum amount of $400.00 covered per person each year.

Prescription Weight Loss Medications

Persons 60% over their ideal body weight (documented by a physician) are eligible. Eligible members will be reimbursed 100% for weight loss medications for a maximum of six months in a lifetime under strict physician supervision. Prescribed medication must be FDA approved for weight loss.

Nutrition/Diabetes/Weight Management Program

Any employee, spouse or dependent (over 13 years) currently enrolled in Lucas County Benefits can participate.  Diabetes, nutrition, and weight management education provided through any one of the various hospitals may be reimbursed 100% through the Wellness Program.
