Important Information Regarding Passports and Travel Outside the United States

To apply for a new U.S. Passport Booklet or Passport Card complete the U.S. Passport Application (Form DS-11) on line and print. Bring the application to one of our Auto Title Branch offices for processing. NOTE: Do not sign the application prior to arriving at one of our offices. We must administer an oath prior to your signing. Follow the link provided below for U.S. Passport Information and mouse over to Forms and click Passport Application DS-11, then Steps to Submit Form DS-11. To renew a U.S. Passport by United States mail, complete the U.S. Passport Application (Form DS-82) on line and print. Follow the link provided below for U.S. Passport Information and mouse over to Forms and click Passport Application by Mail then Steps to Submit Form DS-82. Please read and following instructions carefully. NOTE: You may choose the option of coming to one of our branch offices listed above to renew your U.S. Passport and obtain the required updated photographs. If you choose this option, we are required to charge the full price using a DS-11 form. This will ensure that the application process is accurate and complete and our office will process and mail to the correct U.S. State Department passport processing center.

Please note that two separate payment methods are required as stated above. The application fee must be paid by personal check or money order and made payable to the U.S. Department of State. The Execution Fee paid to the Acceptance Facility (Lucas County Auto Title) may be paid in cash, personal check, money order or all major credit cards.
Bring your completed U.S. Passport Application form DS-11 to any of our four (4) Automobile and Watercraft Title Branch offices listed above for processing, or visit any of our offices listed above and our staff will provide you with the required forms necessary to complete the application process. Do not sign your completed form. We must administer an oath prior to signing. We will take the required photograph(s), process the application, receipt all fees and mail to the United States Department of State's National Passport Processing Center. Remember, when applying for a U.S. Passport, you must provide a certified copy of your birth certificate and one other form of identification such as a valid driver's license or state I.D.
If you were born in the State of Ohio and are in need of a Certified copy of your birth certificate, you are no longer required to contact your county of birth. The Toledo Lucas County Health Department can provide certified copies for any of the 88 counties in the State of Ohio.
