Intersystem Care Coordination

Wraparound is a collaborative, team and strength based approach to service and support planning. This process is individualized and ensures the family's voice and choice is heard. Facilitators encourage teams to remain persistent even when situations are difficult. The goal is to have the youth and family function and be involved in community based services. The families natural supports family, friends, neighbors, and other people who care about the youth and/or family are invited an encouraged to come to the table to plan and help. This occurs all while being culturally competent.
