Appellate Filing Procedure

The following is a general description of the forms and/or procedure
required when appealing a decision in our court.
First, you will need to prepare the Notice of Appeal. You will need to attach
a copy of the Opinion and Judgment Entry. You will also need to file a
Praecipe and Docketing Statement.
You will need the following copies when filing a Notice of Appeal,
Praecipe, and Docketing Statement:
- Original and one (1) copy for Court of Appeals
- One (1) copy for the Clerk's Office
- One (1) copy for the Courtroom
- Copies to each party listed on the Certificate of Service
- One (1) copy for the Court Reporter (if transcript is requested)
Our office needs the same amount of copies of the Praecipe and
Docketing Statement and any additional copies that you would like
returned to you.
