A HEALTH PARTNERSHIP BETWEEN THE BOARD OF LUCAS COUNTY COMMISSIONERS AND ITS EMPLOYEES

WHAT IS THE LUCAS COUNTY WELLNESS PROGRAM?

The Lucas County Wellness Program is a health partnership between the County and its employees. The program is designed to respond to employee’s recognized needs for guidance and provide encouragement, empowerment and education in dealing with areas of health concern. It provides the employee with knowledge to recognize health risks and the means of addressing those risks. It also provides employees opportunities to address wellness needs to improve physical, mental and emotional health concerns.

EMPOWERMENT

Participating in the Wellness Program means devising a reasonable plan to achieve one’ health goals. To assist in articulating and achieving these goals, Lucas County has provided Health Coaches. A Health Coach is not a personal trainer, but will meet with individual employees to conduct an assessment of the employee’s health status to help him/her identify goals, and set up a realistic means of achieving these identified goals.

EDUCATION

As part of the health partnership, the County will make available to employees educational materials designed to bring the latest and most innovative sources of information for making good health choices. Look for the quarterly newsletter, Wellness at Work, from the Health Coaches in your paychecks.

WHO IS ELIGIBLE TO PARTICIPATE IN THE WELLNESS PROGRAM?

All employees, spouses and dependent children age 13 and older, who are enrolled in health benefits through Lucas County are eligible and encouraged to participate.

HOW DO YOU PARTICIPATE IN THE PROGRAM?

Contact a Health Coach at 419-213-2088/2089. Schedule and attend an assessment session with the Health Coach to develop a schedule of wellness activities. If reimbursement is desired, articulate those activities in a Lucas County Wellness Program Performance Contract. Reimbursable activities must be approved in advance by the employee, Health Coach and member of the Risk Management staff. In order to receive reimbursement, the employee is required to provide documentation of contract compliance including documentation of attendance and a receipt of payment.

HEALTH EDUCATION

Brown-bag lunch seminars are sponsored by the Health Coaches at any of the departments. Health screenings (cholesterol, blood pressure and blood glucose) can also be provided. Call the Health Coaches for more information at 419-213-2088/2089.

EXCLUSIONS

Any expenditure incurred that is not included in a Lucas County Employee Wellness Program Contract is specifically excluded from reimbursement under this program. In no case will reimbursement be provided unless the specific terms of the Lucas County Wellness Program Contract are fulfilled. Reimbursement for any personal property is excluded under this Program. All former employees are ineligible for this program. All employees who are eligible for health benefits, but are not actively at work because they have exercised their rights under the Family and Medical Leave Act, are eligible to participate in this plan.
